Intake Appointment
Complete an online application and meet with our Intake Manager to determine which resources are right for you and your child.

Assessment & Treatment Plan
Schedule evaluations and diagnostic assessments to determine whether or not your child may have ASD or related conditions.

Start Services
Coordinate with our team of clinical experts on your child's treatment plan, and determine the appropriate starting plan and a start date for services.
Professional Referrals
HIA welcomes referrals from our community and partners
Referring a client into our program is convenient and streamlined: you or your student’s parent, or your patient’s parent, can simply call our dedicated intake-line, 612-422-8556, to begin the intake process.
Our intake team will provide an overview of the intake process and collect general patient information. Before scheduling an appointment, the patient’s caregiver will receive an intake packet to complete and will be asked to provide specific documentation, including:
- copies of any recent testing (e.g., cognitive/psychological testing; speech, occupational or physical therapy evaluations)
- records of any early intervention services or IEP/504 plans
- past medical, wellness check and diagnostic assessments
This information will assist our team in scheduling the most appropriate and first available appointment in our center. Please note that this information needs to be returned to us before we can schedule the appointment.
